Blank Check always fails with ADP-054?

I just got an ADP-054 adapter to use with M27C4002 EPROMs and the Blank Check always fails... ???
Even if I take the EPROM out and do a Blank Check with no chip it fails.
However taking the adapter out of my GQ-4X and running a Blank Check always passes.
Did I get a bad adapter?
